The community equipment service (LCES) promotes and facilitates independence, rehabilitation and to help prevent unnecessary hospitalisation.
We manage a catalogue of equipment, logistics, distribution and maintenance to support prescribers meet the needs of children in the community.
Community equipment can also be provided by Lincolnshire ICB
Children's community nursing
The community nursing team provides advice, support and direct nursing care to infants, children and young people who need nursing care and are under the care of a consultant peadiatrician
Palliative and respite care and other provision for children with complex health needs
Specialist palliative care nurse, health care and family support workers. Includes assessment and treatment for children at home (PATCH) team.
Emergency care provision
Provides acute paediatric (medical and surgical) emergency departments for children and young people, including those with SEND
Provision for children and young people's continuing care arrangements
Children’s and young people’s continuing care undertake assessments using a Decision Support Tool and gathering supportive evidence, for children and young people who have needs arising from disability, accident or illness that cannot be met by existing universal or specialist services
