If you experienced an incident we attended or are representing a person who has you can request an incident report.
The document will contain identifying details of the location attended. This includes the time and date of the event, the main cause and a summary of any damage. Please see the sample report for further information.
We will request a payment of £75 via invoice or card payment before you receive this document.
To request a report, please complete an incident report application form. If you are acting on behalf of an injured party, please upload a Data Subject Declaration.

Please note, a report can take up to four weeks to be available. A report for a complex incident may take longer.
