A defective paving slab - Find a freedom of information request

Please could you provide me with the date as to when the defective paving slab that was located directly outside the TSB Bank on 203 High Street Lincoln LN5 7AU and marked with a yellow spot was first reported to you or your agents as requiring rectification. I have attached two photos which shows the defective paving stones that was marked by a yellow spot as at 13 July 2023.

1) Please could you provide details of the date the paving slab was reported to you or your agents on any subsequent occasion after being first reported.

2) Please can you confirm the date the yellow spot was sprayed on the paving stone?

3) Please can you confirm the reason for the yellow spot being sprayed and who sprayed it?

4) Who inspected it, what position do they hold, if that is not the County Council which company do they work for?

5) Please can you provide details of the policy which sets out the timescales for repair of defective paving slabs in High Street Lincoln.

6) Please can you also provide the policy that determines when immediate rectification work or safeguarding measures should be taken when paving stones are identified as unsafe

7) Please can you provide details of any relevant risk assessments that relate to the surface of the highway on Lincoln High Street in this location

8) Please can you provide me with copies of any relevance correspondence that relate in any way to the repair/rectification/safety of the paving stone outside the TSB Bank as identified in the photograph

9) Please can you confirm the date of rectification of the paving slab as set out in the photos and who carried out this work?

I can confirm that the information requested is held by Lincolnshire County Council. I have detailed below the information that is being released to you.

1) Enquiry 4184426 was reported 12/07/23 no other enquiry reported this defect.

2) Slab was marked when job number 5721066 was raised 12/07/23

3) Slab was marked to identify it for the job 5721066 raised by a Senior Highways Officer

8) On 12/07/23 enquiry 4184426 was received on the same day the site was visited a nonintervention level defect 0f 16mm was found, while it was not at intervention level a job was still raised 5721066 for a slab reset on a 6 day response.

9) Our Term Maintenance Contractor Balfour Beatty carried out works within the requested 6 day response time frame - 17/07/23
