Feedback forms for unpaid/informal carers - Find a freedom of information request

1 a) Do you have a feedback form you use specifically for unpaid/informal carers?

b) If so please can you provide a blank version of this form as part of this request

I am seeking your feedback form(s) for adult carers - people who support adult friends, family, etc on an unpaid basis

1. Please see attached three forms that Lincolnshire Carers Service use, two used by carers first and the customer experience survey is offered at the front door of the service by SERCO.

Also, all carers are directly asked about whether they feel listened, treated with compassion and respect, that their wellbeing is prioritised, and if they feel informed about their rights and how to access support during their interaction with the Lincolnshire Carers Service.

Lincolnshire County Council (LCC) also has an online webform, which is designed for anyone to use, it then filter those forms to the relevant teams on receipt. People can also contact LCC by email, phone or letter, but again these aren't feedback methods solely for carers.
