4. Do you provide training for those working with people experiencing homelessness in health care services e.g., standards for GP receptionists in primary care and practice managers? 

5. Do you have Pathways/protocols/ services in place to prevent homelessness at discharge from hospital (including mental health and on release from prison)?

1. Lincolnshire County Council (LCC) are partners in the Lincolnshire Homelessness Strategy. The Lincolnshire Homelessness Strategy can be found on the LCC website.

5. There are two hospital discharge leads with this remit. There is a prison release protocol. The District Councils will be able to tell you more about how they support prison release.
