Transitions Team - Find a freedom of information request

Please can you inform me of the Transitions Team Manager and advise on the age range of the people you support within this team.

Lincolnshire County Council don't have a transition team but do have an Intake team who work with young people with a learning disability eligible for adult care age between 18 and 25 whilst they are in education. This is only for young people with a learning disability and they do other work so not specifically a transition team.
