System for Managing Citizen Records - Find a freedom of information request

1. Which system does your staff use to access and manage citizen records? by citizen we mean the individual you serve/look after or care for. e.g Liquid Logic, Mosaic etc.

2. We are interested in understanding, what your organization/s uses to share citizens information between health and/or social care systems and settings. Which suppliers are in use in your organization/s to fulfil this purpose? (If any) examples such as System C/Graphnet/Liquid Logic, MIG, Direct Integration from/between systems.

2. Lincolnshire are currently implementing a Care Portal supplied by Intersystems to link health and care systems

3. End date 5 years from 6/01/14 with Option to extend by 12 months for a maximum of 5 years. Contract must terminate by 5th January 2024
